IKA Personal Health Booklet - Annual renewal

IKA - the Greek national Social Insurance Institution no longer informs insured persons about the annual renewal of their Ατομικό Βιβλάριο Υγείας Atomiko Bibliario Ygeias (personal health booklet).

From 2015, renewal can be done through the local Kentro Exypiretisis Politon (KEP - Citizen Service Center), or online by Internet.

The online method is only available for the Greek IKA Booklet. For Health Booklets from abroad, you need to contact your local IKA office.

For renewing the Greek IKA Booklet, click on https://apps.ika.gr/eAccess/login.xhtml and complete the following form:

• Αριθμός Μητρώου Ασφαλισμένου (Α.Μ.Α.) (A.M.A. - IKA book number)
• Αριθμός Μητρώου Κοινωνικής Ασφάλισης (Α.Μ.Κ.Α.) (A.M.K.A. - social insurance number)
• Αριθμός Φορολογικού Μητρώου (Α.Φ.Μ.) (A.F.M. - tax number)

In the last field, type the Κωδικός Οπτικής Επιβεβαίωσης (Code of visual confirmation) as shown in the frame just above it.
If you cannot read the Code, click Ανανέωση (Refresh) to receive a new code.

After completing the form, hit the Είσοδος (Entry) Button.

On the next screen, move the printer symbol on the right hand side to the row where the Τύπος (Type) is Ιατροφαρμακευτική και Νοσοκ. Περίθαλψη (Medical and Hospital Care), to print the document. Keep the document in your Personal Health Booklet.   Source thecic eu

**UK Pensioners who intend to take up residence in Greece and who are entitled to free medical treatment in the United Kingdom or another EU country, also enjoy a similar entitlement in Greece. They should present the form S1 (previously E121), issued by the Department of Health Office in the UK, to the local IKA office. The documents listed below should also be presented. After successful completeion of the process, they will be issued with an IKA health book.

Documents required for IKA health book (Βιβλιάριο υγέιας) under the reciprocal agreement for form Form S1.
• form S1 ( old E121 & E119
• photocopy of passport
• you may also be asked to produce a residence certificate
• tax number, 2 photographs
• An AMKA number
